- Medication Error Reporting:
- In all situations: complete a district-level incident report and notify the student’s caregiver.
- For guidance on submitting a medication report, see the School Health Medication Error Report Guide.
- Use the School Health Medication Error Report Form to report to MDPH any error that has resulted in a serious illness/injury requiring medical care.
- Submit a Drug Incident Report (DIR) to report drug discrepancies, suspected diversion, or tampering to the Drug Control Program and email MDPH School Health Unit at medication.delegation@mass.gov.
